Aquablation vs HoLEP and Laser Enucleation: How They Compare
By Dr. Ahmed Ali Alnaimi, Consultant Urological Surgeon

For men with a moderate or large prostate, two modern approaches are often discussed: laser enucleation (HoLEP with a holmium laser, or ThuLEP with a thulium laser) and Aquablation. Both are performed through the urethra with no external cuts, and both can give excellent, lasting relief. They work in different ways.
How laser enucleation works
The surgeon uses a laser to separate the enlarged inner part of the prostate from its outer capsule, much like lifting the segments of an orange away from the peel. The freed tissue is pushed into the bladder, broken up and removed. Enucleation removes a large amount of tissue and has long-term evidence behind it. It is a highly skilled procedure, and results depend on the surgeon's experience.
How Aquablation works
The prostate is mapped on live ultrasound, then a robot removes the planned tissue with a heat-free waterjet. Because the surgeon decides in advance what to leave untouched, areas linked to ejaculation can be preserved.
Key differences
- Ejaculation: retrograde ejaculation, where semen passes back into the bladder, is common after enucleation. Aquablation aims to preserve normal ejaculation in most men.
- Tissue removal: enucleation removes the whole enlarged inner gland; Aquablation removes a planned volume and leaves selected tissue in place.
- Energy: lasers use heat to cut and seal; Aquablation removes tissue without heat.
- Bleeding: enucleation seals vessels as it goes, so bleeding is usually low. With Aquablation, bleeding control at the end of the procedure is a key step.
- Very large glands: both can treat large prostates; the best choice depends on size, shape and your priorities.
Which is better?
Neither is best for everyone. A man who places a high value on preserving ejaculation may lean towards Aquablation. A man with a very large gland, or who wants as much tissue removed as possible, may be well suited to enucleation. Dr. Ahmed Ali Alnaimi has experience across modern prostate treatments and will explain how each applies to your own prostate.
Every prostate, and every patient, is different. A consultation gives you a clear picture of your own options.
